We know this often isn’t an easy call to make. You might be worried about Mum or Dad, unsure what help is even available, or trying to sort something out before a hospital discharge. Whatever the situation, you can talk it through with us first. There’s no pressure to arrange anything.
When we’re open
Our office is open 8am to 6pm, Monday to Saturday. Calls and emails during these hours reach the team who plan and co-ordinate care, so you can get straightforward answers about availability, costs and next steps.
Forms received outside our office hours are reviewed when we open at 8am Monday to Saturday. For urgent matters relating to hospital discharge, use our fast-track form and we’ll respond within a few hours.
Our out-of-hours line runs 7am to 10pm, seven days a week, for urgent matters only. If you’re arranging care around a hospital discharge that’s happening soon, the fast-track form is the right route, it notifies our duty team directly.

What happens after you get in touch
- We listen. You explain the situation in your own words, by phone or in the form. We ask about routines, what’s changed and what’s worrying you.
- We check availability honestly. If we can help in your area, we’ll say so. If we can’t, we’ll tell you that too rather than keep you waiting.
- We arrange a care assessment. We come out to meet you and your family member, look at what support would actually help, and explain likely costs clearly.
- You decide. Nothing starts until you’re ready. Care is planned around the person, their dignity and their routines.
